We hope that this general information may help you with the initial steps you will need to take when you suffer a bereavement be it at home, nursing home or a hospital and information on the registration process.
If death has occurred at home
Contact the GP to confirm death & issue the Cause of Death Certificate. If the death is unexpected the coroner may need to be informed, the GP will advise on this.
If death has occurred in hospital
You will receive the Cause of Death Certificate from patient affairs & if death has occurred in a nursing home or hospice, the staff will contact the on call doctor on your behalf.

Decide on a cremation or burial
Check the will to see if any wishes have been stated.
Register the death at the Registrar’s Office
Some operate an appointment system.
When you register you will need to take with you:
- Cause of Death Certificate from the GP
- Payment for copies of the Death Certificate.
The registrar will need to know the following information in regards to the deceased:
- Full name
- Last address
- Occupation
- Date & place of birth
- Details of any surviving spouse or civil partner
- Information of any state benefits
The Registrar will issue you with a Green Certificate for Burial or Cremation
We will need this for the funeral arrangements. You will also be issued with as many copies of the Death Certificate as you need.
